Moving forward, we’re streamlining how to request access to Point of Care instruments!
Instead of sending emails- which can easily get buried in our inbox- please use the form linked above. This helps us ensure every request is received and processed promptly.

When filling out the form, please indicate which instrument you or others need access to, along with the unit where it will be used. You’re welcome to submit one form for multiple operators. If you have a list of names and UMID numbers in an Excel file, you can simply upload it with your request.

If you or someone else previously had access but are now locked out, we’ll grant 2 week temporary access. For the glucometer, you’ll need to run a set of QC. For the Clinitek and Afinion, please run a demo patient using a sample ID of six zeroes (000000). For the Gem5000 or Hemochron, completion of a check-off with your unit’s superuser will be required.

If you need a barcode, just fill out the relevant section on the form, making sure to include a unit contact and the tube station or clinic (for ambulatory locations).
